It can be a rather daunting task trying to find out if you, or an elderly loved one are eligible for any Home and Community Based Services Programs, waivers.
So, I have put together a list of all the available Medicaid and state funded Home and Community Base Services offered by each state, and linked to the appropriate website.
I have added other State Medicaid Programs which also fund assistance to the elderly, so that they may maintain their independence and continue to live in their own homes.

There is a list of State waivers for Medicaid, which you can check on the Medicaid.gov website.
Their list will not include some Personal Care Service Plans, Care in the Community, or State Medicaid Health Plans which are also available from different state governments.
